An 81-year-old woman is admitted to hospital following a fall at home. She has a background of hypertension and osteoarthritis and lives alone. On day 3 of admission, nursing staff note that she is more confused than usual, pulling at her cannula and attempting to climb out of bed. She is alert but disoriented to time and place. She is currently on IV fluids and co-codamol for hip pain. Her observations are within normal limits. You are the junior doctor reviewing her on the ward round.
