He has a history of hypertension, type 2 diabetes mellitus, and hyperlipidemia but has not been compliant with his medications. He smokes 20 cigarettes per day and drinks alcohol occasionally. There is a family history of ischemic heart disease (his father had a myocardial infarction at 60 years old).
